- My vendor registration has expired or is no longer active. How do I re-register as a vendor?
- The re-registration process is essentially the same as the registration process.
- Log into MFMP to make sure your registration is still active and that they have the correct address. Make any necessary changes.
- Register your electronic W-9 with the DFS, if you have not already done so.
- Submit the appropriate application and any supporting documents that may be required.
- How do I change my Tax ID number?
- Follow the How to Become a Vendor instructions. Complete the three-step process for the new Tax ID number that you wish to operate under:
- MFMP registration
- Electronic W-9 registration
- New VR Application Submission. The VR application includes a cover sheet. The coversheet has a space to indicate if you are changing or adding a tax ID number as well as a section to indicate what you would like to do with your existing Tax ID number.
- How do I change my vendor information (e.g., address, contact information, business or individual name)?
- Follow the detailed instructions found on the Vendor Information Update Form. This process consists of two steps:
- MFMP Update
- Submission of the Vendor Information Update Form
- I am already a VR vendor but I want to provide a different service in addition to what I am already providing. How to I become registered to provide additional services?
- Submit the appropriate application and any supporting documentation that is required for that vendor type. This information can be found in the Vendor Qualification Manual.
I no longer wish to be a vendor with VR. How can I withdraw and terminate my VR registration or a registration for a specific individual who no longer operates under this Tax ID/SSN?- Download the Vendor Withdrawal Form and submit it to the Vendor Registration unit. You will be notified when the withdrawal process is complete. If you wish to register again, you will need to complete the entire New Vendor Registration process again.
My company and/or I are already registered as a vendor. How can I add an additional vendor to be registered under the same SSN or Tax ID Number as myself and/or my company?- The vendor being added will need to complete the appropriate VR Vendor Application. On the application, there is a section that asks for individual’s name. In this section, write the name of the vendor being added. Submit the vendor application along with any required documents (if applicable) to the Vendor Registration unit. The application will be processed and a notification will be sent once complete.
The application I am required to submit needs to be notarized. Does that mean that I need to submit it by mail or may I fax it?- You may fax it. Scan and email it to the Vendor Registration Unit.
How can I get information about a VR vendor’s payment history or recent payments?- Access the state of Florida’s Vendor Payment History page.
Note: The state of Florida requires its vendors to submit a Substitute Form W-9 to ensure accurate federal tax reporting. Effective March 5, 2012, vendors will not receive orders or payments from the state if a valid Substitute Form W-9 is not on file. Go to the State of Florida Vendor Website to register and complete your Form W-9.
Who should I contact if I have a vendor payment discrepancy, question or issue?- All payment-related questions should be referred to the VR counselor whom you have been working with or to your local VR Office. You can find the contact information for local VR offices.

- What is Vocational Rehabilitation?
- Vocational Rehabilitation (VR) is a federal-state program that helps people who have disabilities get or keep a job. VR is committed to helping people with disabilities find meaningful careers. On-the-Job Training (OJT) is one way of preparing VR customers for success in the workplace.
What type of services does VR provide?- Vocational evaluation and planning; career counseling and guidance; job coaching and placement; on-the- Job training; training and education after high school; job-site assessment and accommodations; supported and customized employment services; Pre-ETS, assistive technology and devices; medical and psychological Assessment, and time-limited medical and/or psychological treatment.
How much does VR pay vendors?- For health care services, VR pays the amount payable under the Medicare Part B system. For hospital per diem payments, VR pays the amount payable under the Medicaid system. In setting its highest allowable fee for all other services, VR shall ensure the fee is not set so low as to deny individuals the right to make an informed choice.
How do I register as a VR vendor?- Vendor registration is a three-step process which includes registration with MyFloridaMarketPlace, sub-mission of an electronic W-9 with the Department of Financial Services and registration with VR. Visit Steps to Become a Vendor for more information.
Are all vendors required to have a background screening?- Not all vendor types are required to have a background screening. Visit the Background Screening web page for information on requirements.
How long does the vendor registration process take?- If all three steps of the process have been completed, the standard processing time is approximately 10 business days. Vendor types requiring background screening may take longer. Note: If you are applying for an Employment Services Contract and/or Pre-Employment Transition Services, the time frame for processing is 6 to 12 weeks.
How do I check my application status?- Allow for the standard processing time of 10 business days before inquiring about your vendor status. Send an email for status requests to VRVendors@vr.fldoe.org or call 866-580-7438 or 850-245-3401.
How will I know if I am approved as a VR vendor?- Once your application is processed completely and a registration determination has been made (Approved, Denied or Incomplete), you will receive notification of your registration status.
Once approved, what is the next step? How do I get client referrals?- As a new vendor, introduce yourself with an overview of your services to your local VR offices. Your local offices will provide the details on how they authorize for services. VR does not guarantee referrals.
